At Trustline Care, we provide compassionate and dignified end-of-life care, ensuring individuals are comfortable, respected, and well supported as they approach the final stages of life. We also recognise the emotional impact this time has on families and loved ones, and our team is there to offer reassurance, guidance, and practical support throughout.

End-of-life care is a deeply sensitive subject. When possible, planning ahead can provide clarity, comfort, and peace of mind for everyone involved – the individual, their family, and those providing care. Our role is to support these conversations with care, sensitivity, and professionalism.

 

Planning Ahead for End-of-Life Care

Advance planning allows individuals to express their wishes clearly and helps ensure care is delivered in line with their preferences. Trustline Care supports individuals and families to plan ahead in a way that feels right for them.

Creating an Advance Statement

An Advance Statement is a written document that sets out a person’s preferences, wishes, and values in relation to future care. Our team can support individuals to create an advance statement, which may include:

  • Where the individual wishes to receive care, such as at home, in hospital, a care home, or a hospice
  • Practical preferences, including personal care routines, daily living support, and arrangements for dependants
  • Religious, cultural, or spiritual beliefs that should be respected and reflected in care

Advance statements help ensure that care remains person-centred, even if the individual becomes unable to communicate their wishes in the future.

 

Advance Decisions (Refusing Treatment)

An Advance Decision (sometimes known as an Advance Decision to Refuse Treatment) is different from an advance statement. It allows an individual to formally refuse specific medical treatments, including life-sustaining treatment, in certain circumstances.

An advance decision must:

  • Clearly state which treatments are being refused and in which situations
  • Be written, signed, and witnessed
  • Be made when the individual has mental capacity

Trustline Care can support access to appropriate healthcare professionals who can explain treatment options, why they may be offered, and what it means to refuse them, enabling individuals to make informed decisions.

We understand that end-of-life care affects not only the individual but also their family and friends. Those close to the person often need emotional support, reassurance, and guidance during this time.

Our team ensures families and loved ones are supported, listened to, and not overlooked. We provide compassionate support to help them cope with the emotional challenges of knowing a loved one is approaching the end of life.
